6. Employer understands that the services provided by all caregivers engaged through MONTICELLO
NANNIES must be contracted through the MONTICELLO NANNIES
Offices and that ALL
communication with caregiver engaged through MONTICELLO NANNIES
should be through the
Agency. Employer agree that he/she will not hire or refer a MONTICELLO NANNIES caregiver to a
Third Party withough the consent of the MONTICELLO NANNIES
Offices. Employer further
understands that use of the agency is considered “Theft Of Services”, and will result in a charge
of One Thousand Five Hundred Dollars ($1,500.00) for liquidated damages. In the event
Employers does not hire or retain the services of a nanny candidate submitted to Employer by
MONTICELLO NANNIES
and Employer refers the nanny candidate to a Third Party who hires or
retains the services of the candidate, Employer will pay to MONTICELLO NANNIES
the entire
placement fee under the same terms and conditions as if Employer had hired or retained the
services of the nanny candidate.
Likewise, MONTICELLO NANNIES
will not disclose to any Third Party, other than is necessary to
perform services pursuant to the Services Agreement, any personal or identifying information
provided to MONTICELLO NANNIES
by Employer. MONTICELLO NANNIES
DOES NOT store or enter
your credit card information in our computers of the Internet. All transactions are secure from our
office directly to our Merchant Service Provider.
7. APPLICATION PROCESSING FEE
Monticello Nannies requires an application processing fee of $300. This fee is needed to complete background check and DMV checks. To process the application fee, please contact Angela Burruss-Terrell @ (434) 996-4479.
8. FEE SCHEDULE
In the event that the Employer wishes to hire a caregiver referred by MONTICELLO NANNIES
on a
permanent basis, the standard placement fee will apply less the amount paid by the Employer to
register for Temporary/On-Call Srvices, pro-rated by the month. MONTICELLO NANNIES
standard
placement fee for a permanent caregiver is fifteen percent (15%) of the nanny’s projected annual
compensation.
The Employer understands that caregiver working with MONTICELLO NANNIES
are ‘Associates’, and
NOT ‘employees’, of MONTICELLO NANNIES.
The Employer understands that if they pay more than
One Thousand Six Hundred Dollars ($1,600.00) per calendar year to any one (1) caregiver, the
Employer is responsible for withholding and filing employment taxes for that person. Social
Security numbers of caregiver must be obtained directly from caregivers.
9. EMERGENCY MEDICAL CARE RELEASE
In case of a medical emergency, the MONTICELLO NANNIES
caregiver will use all reasonable efforts
to contact me. Should such efforts to contact me fail, I hereby grant permission to the caregiver
to secure emergency treatment for my child/children as may be necessary in my child/children’s
best interest.
9. Employer agrees to forever indemnify,release and hold harmless MONTICELLO NANNIES
caregivers
as well as MONTICELLO NANNIES
its agents, employees, and officers for any and all claims relating
to services provided by caregivers.
